What this is
The Common Services Agency (Public Services Delivery Scotland), on behalf of NHSScotland, is exploring the potential procurement of a multi-channel remote health monitoring (RHM) and communication solution to support enhanced care pathways, including home and mobile health monitoring. RHM lets patients record and share clinically relevant health information from home so care teams can monitor status, support self-management and intervene proactively. Connect Me is NHSScotland's national RHM service, currently delivered on the Inhealthcare platform, supporting collection of measurements and patient-reported information via automated calls, SMS and web/mobile channels. This is a Prior Information Notice serving as a soft market test to assess options, service continuity, migration approaches and indicative costs.
