What this is
Social Care Wales seeks a supplier to design and develop a coherent, evidence-informed Multi-Disciplinary Team (MDT) support offer for Welsh local authorities. The work includes research, stakeholder engagement, co-design, mapping of existing provision, testing and feasibility assessment to create a practical, scalable support model with implementation pathways for national rollout by autumn 2027.
