What this is
West Dunbartonshire Council, for the West Dunbartonshire Health and Social Care Partnership, is establishing an Open Flexible Framework (Foundations for Change) for community support services for adults aged 18-65 with additional support needs — learning disability, autism, complex needs, physical disability, cognitive impairment including acquired brain injury, sensory impairment and mental health challenges. Delivered via Self Directed Support Options 2 and 3 with a person-centred, outcomes-focused approach.
