NHS South East London Integrated Care Board is seeking to commission a Neighbourhood Homeless Health Hub Service for the London Borough of Bromley, building on a three-year partnership between Bromley GP Alliance and Bromley Homeless Charity. The service provides trauma-informed, integrated healthcare for people experiencing homelessness via a hub-and-spoke model, including drop-in clinics, extended appointments, outreach, long-term condition management, mental health support and safeguarding, aiming to reduce avoidable A&E attendances and health inequalities.
